New Patients
Welcome New and Prospective Patients!Thank you for choosing Woodinville Physical Therapy for your rehabilitation needs. Below you will find the necessary paper work that we will need on your first visit with us. Please download, print and fill them outcompletely and bring with you to your appointment. If you forget to bring this paperwork with you it will have to be completed before your visit. Please arrive 5-10 minutes early to allow us time to put your chart together and get the necessary information in order, so you will be ready to see the Therapist at your scheduled appointment time. The following is what you will need to bring for your visit regardless of what type of insurance we are billing for you:
- The written doctor’s prescription, current referral for physical therapy.
- Your medical insurance card, claim number and any other pertinent billing information, along with a piece of photo identification.
- Comfortable, loose clothing (shorts and/or tank top, T-shirt. We do have shorts available if needed.)
- Your daily and recreational shoes, if appropriate for your diagnosis.
We do recommend that you contact your insurance company to verify your benefits, so that you are aware of your insurance coverage for physical therapy. We also do appreciate you giving us 24 hours notice of any cancellation.
