Coverage and benefits vary by plan, we recommend contacting your insurance company before your first visit to confirm your physical therapy benefits and any copays, deductibles, coinsurance, or other out-of-pocket costs.
We do not bill out-of-network insurance plans. If we are not in-network with your insurance, you may still have the option to receive care through our self-pay rates.
You are responsible for any amount your insurance determines you owe, including deductibles, copays, coinsurance, or denied services.
Insurance co-pays and self-pay rates are due at time of service.
Please let us know as soon as possible if your insurance changes. Providing updated information helps us submit claims correctly and avoid delays or denials.
If we are not notified of an insurance change and a claim is denied because we did not have current insurance information, you may be responsible for the resulting balance.
