The week before you initial appointment you should receive an email asking you to fill out the paperwork. This is all online and is better done from your computer or tablet than a phone. If you have technical difficulties you can fill it in on the tablets in our clinic, we just as that you please arrive 20-30 minutes early to allow yourself enough time so we can get the necessary information so you will be ready to see your therapist at your scheduled appointment time. The following is what you will need to bring for your visit regardless of what insurance we are billing for you:
We do recommend that you contact your insurance company to verify your benefits so that you are aware of your insurance coverage for physical therapy.
We require a 24 hours notice prior to any cancellation or their will be a late cancelation fee.

- Your medical insurance card, claim number and any other pertinent billing information.
- Photo ID
- Comfortable, loose clothing (shorts and/or tank top/t-shirt) will be helpful for physical assessments. We do have shorts available if needed.
- Your daily and recreational shoes, if appropriate for your diagnosis.
- The written doctor’s prescription, current referral for physical therapy, if needed (required for Medicare patients).
