To schedule your first appointment, please call us at (425) 481-1744.
Preparing for Your Visit
Online Paperwork
The week before your initial appointment, you'll receive an email with a link to complete your new patient paperwork online. For the best experience, we recommend using a computer or tablet rather than a phone.
If you encounter any technical difficulties, you're welcome to complete the forms on our clinic tablets. If you plan to do so, please arrive 30 minutes early for your appointment to ensure you have enough time before seeing your therapist.
If paperwork is not completed, or you do not show up at least 30 minutes early to complete it, your appointment may be canceled and rescheduled to another date.

Please bring the following items to your first appointment:
Cancellation Policy
We require at least 24 hour notice for appointment cancellations. Appointments canceled with less than 24 hour notice are subject to a late cancellation fee.
For initial evaluations, a $50 deposit is required to reschedule an appointment that was canceled with less than 24 hour notice. This deposit will be applied toward any copay, coinsurance, or deductible that you are responsible for according to your insurance. If your insurance determines that you have no patient responsibility for the visit, the $50 deposit will be refunded to you.
If you do not attend your rescheduled appointment or cancel it on the same day, the $50 deposit will be forfeited and applied as the late cancellation/no-show fee.
Insurance Verification
We recommend contacting your insurance company directly to verify your physical therapy benefits. This will help you understand your coverage and any potential out-of-pocket costs.

- Medical insurance card, claim number, and any other relevant billing information.
- Photo ID.
- Comfortable, loose-fitting clothing (such as shorts or a tank top/t-shirt) for your physical assessment. We have shorts available if you need them.
- Your daily and recreational shoes, if applicable to your diagnosis.
- Your written doctor's prescription or current referral for physical therapy, if required by your insurance (this is required for all Medicare patients).
